BRACE Image Consent Form

By completing this form, I grant BRACE permission to use photographs and/or video recordings containing my image, likeness, and/or voice for promotional, marketing, educational, and publicity purposes across all media platforms including but not limited to websites, social media, printed materials, presentations, and advertisements.

I understand that these images may be edited, copied, exhibited, published, or distributed without further approval from me, and I waive any right to inspect or approve the finished product. This consent remains valid unless I submit a written revocation to [email protected].

I confirm that I am at least 18 years of age and that I have read and understood the terms of this agreement before signing.
